What method of payment is accepted?

 

I accept cash, checks, and insurance payments.

What insurance plans do you accept?

 

Blue Cross Blue Shield PPO and DSCC. Other insurance plans are acccepted, but at the out of network rate.

Will you do the billing for my insurance?

 

Yes, I will submit an itemized statement to your insurance company. Please be aware that you are responsible for any amount that is not covered by insurance.

Do you provide Early Intervention services?

 

Yes, but on a limited availability and within a limited proximity to my office.

What happens if I need to cancel an appointment?

 

If you need to cancel, simply call me twenty four hours before the scheduled appointment. I understand that issues come up so just please notify me as soon as possible via e-mail or text to my phone number.

Will I be present for my child's evaluation?

Yes, I recommend that the parent is present for the evaluation.
